FBHS Prescription Renewal Policies
If the following procedures are not used, your prescriptions WILL NOT be renewed. If you are not following your doctor’s treatment plan for follow-up appointments, your prescriptions WILL NOT be refilled.
Prescriptions are only refilled and picked up Monday - Friday 9:30 a.m. and 4:30 p.m.
Controlled substance prescriptions (Ritalin, Adderall, Concerta, Focalin, Vyvanse, Etc.)
Patients who are on a controlled substance must be seen every 1-3 months depending on your treatment plan set up with your doctor. If you NO SHOW and/or CANCEL an appointment without proper excuses or notice it will prohibit refills until an appointment is completed with the doctor.
Medications will NOT be replaced if they get lost, get wet, are destroyed, left on an airplane, etc., so protect your medications. If your medication has been stolen and you complete a police report regarding the theft, an exception MAY be made once. (at the doctor’s discretion)
Prescriptions are to be used ONLY as written Use of increased amount of medication, without consultation with a physician, will not be allowed and may lead to immediate termination.
You may not share, sell, or otherwise permit others to have access to these medications.
Controlled substance RX’s can only be written.
For written prescription refills: Allow 5 business days for request
To request a refill, call your doctors voicemail, email or fax our office.

Please be sure to include the following information:
Patient’s name, Date of birth, 90 or 30day RX, full name of medicine amount you are taking and the directions of how you take it.
Refills directly to a pharmacy: Allow 2 business days for refills
We do not call the pharmacy; they must fax us.
You must have your pharmacy fax our office 440-460-5413 a refill request.
You must set up with your pharmacy yourself to have prescriptions faxed to our office, we will not initiate faxed prescriptions.
Please check with your pharmacy after 48 hours, we do not call you back to tell you the prescription was called in.
